SB-CEU APPLICATION

 

If you are conducting a program and would like to offer your class SB-CEUs, you must:

 

1.  Complete the application. (Link below)  This is a word document, so you will be able to type in it, save it, and email it to your district contact person for SB-CEUs.

 

2.  Your district contact person will send it to CISD.

 

3.  If everything is correct, the CISD will apply to the MDE. If changes need to be made, you will be contacted.

 

 4.  When your program is approved, you will receive all of the forms necessary to conduct your program.
